* In case of a scholarship or official invitation present evidence plus copy

Marriage/Settlement:

Required Documents for Passport Application

Lady (reaching her age of consent of 20 years)
1. Identification Card or Substitute (Bor.Por.2) affixed with photograph and official seal, Official Employment Card, State Enterprise Employment Card, Municipality Employment Card.
2. Deed Poll (Name Change Certificate) (if applicable)
3. Marriage Certificate or Divorce Certificate (currently valid)
• Marriage Certificate - The title of courtesy stated in Identification Card and House Register must be amended to "Mrs." And adopted the husband surname.
• Divorce Certificate - The surname stated in Identification Card and House Register must be amended to the original maiden name.
4. House Register
5. Child Adoption Certificate (if applicable) or Revocation Certificate of Child Adoption (if revoked)
6. Naturalization Certificate or The Government Gazette (if not born in Thailand) or Thai National Certificate (in case of born in Thailand but hill tribes)

Gentleman (reaching his age of consent of 20 year old)
A. Identification Card or Substitute (Bor.Por.2) affixed with photograph and official seal, Official Employment Card, State Enterprise Employment Card, Municipality Employment Card.
B. Document as per Clause 2, 4, 5, & 6 above
C. Evidence of Military Service e.g. Sor.Dor. 1, Sor.Dor.8, Sor.Dor.9 or Military Conscription Certificate (for age 17 but not exceeding 45 years)

Minor Child (age not exceeding 20 years)
1.1 Birth Certificate (infant age not exceeding 15 years) Identification Card or Substitute (Bor.Por.2) affixed with photograph and official seal (age above 15 years of age)
1.2 Male minor child at the age of 17 years or above is required to produce Military Conscription Certificate as specified in Clause C above
1.3 Deed Poll (Name/Surname Change Certificate) of the minor child and parents
1.4 House Register of the minor child and parents
1.5 Identification Card of parents (Valid Alien Registration Card or Passport of parent in case
of foreign national) and Marriage Certificate or Divorce Certificate (as per Clause 3
above)
• In the event of divorce and right of custody has been set forth, father or mother who has been granted the right of custody is entitled to apply for the child passport and Divorce Memorandum issued by the district is required *
1.6 In case of adopted child, adopted parents of the adopted child are exclusively entitled to apply for the child passport
1.7 In case of signatory for the application, both parents are required to subscribe their names in the application In the event that one of each parents is accompanied the child for the child passport application, a Letter of Consent for the absent parent issued by the district together with document set forth in Clause 1.3, 1.4, 1.5, 1.6 is required
1.8 Required document as per clause 6 (Minor Child, Father, Mother)

Additional Information In Connections With Passport Application
2.1 ID. Card Substitute (Bor.Por.2) must be accompanied with the previous I.D. Card
2.2 Original I.D. Card must not be double-laminated (In case of double-laminated, a forgery will be applied)
2.3 All documents submitted must be of original document with its clear copy
2.4 In the event of copy, Certified True Copy by the district must be affixed prior to a submission (This Certified True Copy will allow a holder to use such document as original)
2.5 In the event that the father is of Chinese national, Deed Poll from "Sae" (if applicable) must be accompanied with the application. In case of "Sae" surname had never been used, Birth Certificate is required.
